If you’re beginning to explore new home and living options—or your current living situation has recently changed—you may need to complete a Home and Living Supporting Evidence Form. This form, which can be lodged up to 100 days before your plan ends, outlines your housing goals, current circumstances, personal strengths, and the type of support you continue to require.
Alongside this, a Roster of Care is also prepared. This document, informed by reports from an Allied Health Professional and your Support Coordinator, details the level of assistance you need. It explains how your disability affects areas such as mobility, self-care, and cognitive functioning, and helps determine the most appropriate housing setup for your needs. An Occupational Therapist usually completes this assessment, which is often funded through the Improved Daily Living category of your NDIS plan.
Once everything has been submitted, the NDIS reviews the information to decide on the most suitable funding arrangement to support your goals and help you stay connected with your community. When a potential housing option becomes available, your Support Coordinator will then prepare a Housing Solutions Report to confirm your suitability for supported living.
If you’re thinking about applying for SIL and don’t know where to begin, the CoDesign Care team is here to guide you through the process.

You can also access our accommodation through individually funded support packages such as icare, workers compensation, and insurance-based core supports. These services can be funded in a variety of ways—agency-managed, self-managed, or a mix of both—depending on what works best for you.
